New Patient Registration

man_working To register with the practice please complete the two forms detailed below or call into the practice to collect them for manual completion.

YOU MUST COMPLETE BOTH REGISTRATION FORMS (detailed below) which includes details of your previous practice and if you have any current health concerns.

These forms (2) can be collected from the reception desk prior to your appointment for completion to save time or downloaded from the following links.

We will only offer you an appointment if you have an immediate health concern, or require regular medication. Please ensure you obtain at least one months supply of any medciations from your current GP Surgery to keep you going until registration is complete.

FORM 1

New Patient Registration Form

Please complete this form online, save to your desktop and email to

nhsh.gp55605-admin@nhs.scot

One form is required for each patient registering. Alternatively you can print the form and either drop it off at reception or post to us.

This form must be accompanied by the Additional Information Form (below), again, one for each patient registering.

New Patient Additional Information Form for Registration

One form is required for each patient registering. You can complete this online and save to your desktop and email to nhsh.gp55605-admin@nhs.scot along with the registration form above, or print and post/drop off to surgery.

Please can you also bring with you two forms of identification - one being photographic and one confirming your address. This is not required for children who are registering with their parents or guardians. We are required to sight this evidence to confirm your details but we will not retain copies of this information.
