Care Navigator

(September 25)

 

Hours of work:         Core hours 0900-1300

                                20 hours per week with extra hours to cover absence as required.                

                                   

Responsible to:        Practice Manager

 

 

Job Description

The Care Navigator is one of the administrative team and contribute to the daily office routine.  You will cover the reception area, switchboard and back office on a rotational basis and any administrative duties as directed.      

 

 

Purpose of Role

 

  • To cover reception and answer telephone on a rotational basis during surgery hours
  • Signpost patients to the most appropriate care provider both within and out with the surgery
  • Support and assist clinical staff in any administrative task as required
  • To be involved in the training of students and registrars
  • To undertake summarising of clinical notes and typing of clinical letters and referrals as directed
  • To undertake any other duties as requested

Salaried GP

 (October 2025)

 

Hours of work:          6 sessions a week

Opportunity to cover extra sessions for partners during periods of absence for example annual leave and sickness

 

Responsible to:        GP Partners

 

Role summary:          The post-holder will provide general medical services managing a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients

 

 

Annual leave:             Six weeks annual leave plus 10 public/ bank holidays (pro rata)

 

Study leave:               Up to two weeks per annum (pro rata)

 

Salary:                        Negotiable and in line with GPC model, depending on qualifications and experience

 

Main purpose of the role:    To support current GPs, to manage a busy and interesting clinical caseload. The appointee will also have the opportunity to take part in leading on key disease areas (monitoring trends in treatment, staff training and prescribing) and assist in developing appropriate healthcare services for a diverse, patient list.
