GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, we will be able to offer you:
- renewal or ordering of repeat prescriptions online

How to sign-up
Please download blank form below, then complete it and bring it to the surgery or when you next have an appointment. Please bring with you a piece of photo ID (passport, driving licence) and something with your address on it, like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up.
You can also collect a blank form from your local Pharmacy.

We acknowledge that it may be inconvenient to have to re-register for this system, but the previous system was independent of your medical record and your details therefore cannot be migrated across to the new system. There are also regulations regarding Data Protection that we have to adhere to. Thank you for your support.
